How do I set up a voicemail and how do I remove a voicemail symbol from my phone?

usually the Voicemail number can be set by click Reset network settings
or you can manually set it yourself using the Settings on your Dialer app. Look for the Voicemail number section on the settings on your dialer app, then set it to one of the numbers on this page:

Once you activate the SIM…it is registered. Just dial your number for voicemail options.

This only means that someone called you and that the person was directed to your voicemail. It's not actually referencing any outgoing call that was made.

I don't believe that's what the customer is trying to do. Disabling these notifications would prevent the customer from knowing about new messages. This is likely an issue of a customer trying to clear the icon after it didn't clear properly after after already listening to all new messages.

The vm symbol should disappear after you delete the messages.
