did you ever set a PIN for sim card? if not, it won't be locked out of the blue
can you show us what the message was when asked the PUK? there are different messages and it means different things

you are certain it is a Sim card lock? what was the error message?
Try the sim card on another phone to see if it works, that will confirm if the lock is on the sim card or if it is in fact a device lock. If it is a device lock, PM support won't be able to help
