It doesn't work 

and do you have a Samsung phone? you might want to check with Samsung at  1-800-726-7864 for a device PUK code.

better try the sim card on another phone first and see if it works

but of course, you can also open ticket with PM support and have them to check if the PUK code they provide can help

did you ever set a PIN for sim card?  if not, it won't be locked out of the blue

can you show us what the message was when asked the PUK? there are different messages and it means different things

you are certain it is a Sim card lock?  what was the error message?  

Try the sim card on another phone to see if it works, that will confirm if the lock is on the sim card or if it is in fact a device lock.  If it is a device lock, PM support won't be able to help
