you sure the PUK is for the sim card? have you been using a SIM PIN and got locked? If it is, then PM support can help
If this phone was never used on PM network, then you are not looking for sim PUK, it is the device puk you need. But you will have to call your old provider, where you got the phone from, for the PUK.
