This is a public forum, you might want to click on the 3 dots to remove your phone number.
For the PUK, please open a ticket with Mod
To open a ticket with mod, click on the Bubble or directly at : https://publicmobile.ca/chatbot
Enter your question in SIMon. Follow the prompts until you reach the Contact Us button
If you have problems submitting a ticket, you can also send a private message to the moderators (but this can take longer):

what exact error was on your phone?
What brand and model is your phone?

You'll need to get the SIM PUK code from the provider, Public Mobile. Since we're users helping users, you'll need to contact a Moderator through the SIMon chatbot (chat bubble, lower-right corner) - best way because you can authenticate yourself with the Ticket. Or, you messaging them directly can be done here:
Just a note - since we're users helping users here... don't post any personal information because we don't want you, your service, or your account to get compromised. 🙂
Edit: If your phone came from another provider, @Dunkman makes a good point, that you'd need to get that unlock code from the original provider who locked it. (eg. If your phone is locked to Rogers, you'll need to contact Rogers for the unlock code.)

What is the exact message shown on your phone? What were you doing when this message happened?
Sometimes it means that your phone is locked to previous carrier.
