Click on the below and chat with SIMon chatbot writing only, "need PUK"

From there, follow the exact instructions the chatbot gives you.

@cholon13 what was the exact PUK you are looking for?
if it is SIM Card PUK, PUK of SIM PIN, you can open a ticket with direct link here:

**Monitor your Community inbox (envelope icon on the top right) after the ticket is opened. CS Agent will reply you there
If it is "SIM network PIN blocked. Enter SIM network PUK." and you have a Samsung phone, you have to contact Samsung Support at 1-800-726-7864
It doesn't sound like it , but if it is "SIM network unlock PIN", then it is a carrier unlock code it is asking for and you need to contact your previous provider for the unlock PIN
