@khadija9513 please remove the phone number, this is an open forum and you don't want to post it here for your own protection
For sim card, get it from Telus or Koodo stores, do not buy online from PM as it could take long for them to deliver
Once you got the sim card, just login to her My Account, then click Change SIM card from the Quick Link menu on top right, follow the steps to complete change

Hello,
my wife is out of the country and she lost her phone. She will need new SIM card with the same number she have.
she asked me to get in touch with you to ask if she can have new SIM card and how she can be able to get it ASAP or if she can order one and pick it up on Wed as she gave some Dr apt and need to have the sim by Wed if possible.
thank you very much for the help

@khadija9513 If it is SIM card PUK, you can use this direct link to open ticket:
https://urlshortner.tiia.ai/Oyn1L6
However, if it is other kind of PUK, PM Support would not be able to assist

Is this a new activation? Or have you been with PM for awhile?

Find your PUK: https://www.publicmobile.ca/en/on/get-help/articles/where-to-find-your-puk
- SIM network PIN blocked. Enter SIM network PUK." is asking for the phone’s (often Samsung) “master code" after entering the wrong code too many times. Samsung and/or your previous carrier can give you this.
- SIM network unlock PIN" is asking for your previous carrier's unlock code - just contact them with your IMEI (dial *#06# to get it) and they'll give it to you. If you enter this incorrectly too many times, you'll get the error message above.
- PIN unlock key (PUK) is a SIM-specific code used to prevent unauthorised use of a SIM card.
The PIN (Personal Identification Number) enables secure access to the SIM card and network. It is provided with your SIM card.
If the PIN code is entered incorrectly 3 times, the phone is locked (except for emergency calls) and requires a PUK (Personal Unlock Key) to reactivate it.
Caution! If the PUK is entered incorrectly ten times in a row, the SIM card becomes unusable.
