As stated it could be several different factors. What model of phone do you have?

It could be one of many factors including @BKNS27  suggestion. The normal signal strength you see at the top of your phone's screen is the 4G LTE network try switching to the 3G network only to see if your signal strength improves. You can get a much more accurate reading by going to device's Settings>>About phone>>SIM card status>> Signal strength.....? Any reading below -99dBm is poor. Ideally you want to see a reading between -60 dBm and -90 dBm.

Of course running thru some basic troubleshooting tips is advisable such as :

  1. Reboot your phone.
  2. Toggling airplane mode on/off.
  3. Remove SIM card. Test SIM card in another phone and/or wait a full 10 minutes before reseating the sim and rebooting.
  4. Reset your network settings.
  5. Log into your self serve and make a $1 manual top up payment by choosing "other amount". Confirm and submit your payment. Log out and reboot.
  6. Log into your self serve and go to the plans or usage page. Click on lost/stolen. Suspend your service. Log out/in. Resume your service. Log out and reboot.

Note: #3, #5 and #6 are specifically aimed at reprovisioning your sim card. Whereas your issue is likely just poor signal strength related to changes to your local cell towers due to upgrades, maintenance, new construction etc....more info on your location could prove helpful to identifying the issue.
