Lost phone, on prepay - never set up self serve

I lost a phone with a SIM card I got at Walmart.  I never set up the online self serve and I want to cancel the prepay on it.  When I try to set up the self serve now it says it will text me to confirm but I don't have the phone any more.  Is there any way to cancel the prepay on the lost phone?

Same thing with links included.  The moderator team can be reached via private message using using smartforms which is initiated by clicking on the ? on the lower right corner of the webpage or try using this link. The guided process will facilitate documentation of the issue or concern and the required resolution which will be submitted to the moderator team.
