I want to ask about this $15 add-on

400 min Intl Long Distance (Incl Canada & US)

Can anyone tell which countries are included in the add on? and how does this add on work?

Dial 011, 49, and then the phone number.

You need to buy the add-on before making LD calls. 1 add-on min is deducted for each min airtime used, regardless of which country. Add-on mins do not expire.
