I got charged for 4 blocks of 4Gb data, but I never ask for them.
In the app I didn't see this option in add-on but via sms I once received a message to send a code to buy a 4Gb data block. How to block add-on from being charged to my account ?

No way really to stop these offers.
The work around would be to remove your payment method from your account. This would mean no automatic payment each month. You would then need to pay manually each time or purchase vouchers. This could end up been difficult to keep up with so that you don't miss payments hence your phone possibly going into suspension.

they send you the data add-on purchase via SMS when you run out of data. you got it because you replied Yes. Was the phone used by another person, maybe your kid? if so, they reply Yes and hence you got charged
you can block that shortcode so you won't receive the text anymore
for the data add-on , I believe you already using it so you won't get any refund. But if you have not use the data add-on yet, you can check with PM and see if they can reverse the purhcase
Simply open ticket with PM using the Orange Chatbot icon on the lower right. (For 2FA code when login, use email to receive code instead of SMS if you cannot receive the text on the phone.) When open Chatbot ticket, Type the question Submit ticket and select Contact Us to get to ticket open screen
