A payment was applied to my account without my authorization. I would like that reversed and account closed effective immediately

what you meant by the payment was applied to your account without your authorization? 

do you know when you joined, you explicitly clicked a box and it will enable Autopay on your account and PM will charge you every 30 days.  You won't be able to activate if you didn't click the box.  So, you have provided your consent to PM to charge you with Autopay.  

If you don't like Autopay, after you activated, you need to login My Account, go to Payment page, click Manage Subscription and disable "Subscribed" (means Autopay).  Did you do that?  If you didn't , then PM was right to charge you.  

PM is a prepaid carrier and do not provide refund in such case.  So, to avoid further charges, please go to the page I mentioned above and disable it

you'll need to message Customer Support to deactivate the account. You likley won't get a refund as it was up to you to close the account by disabling payment method (Subscribed tab)
