What was the exact error message? Depends on the message, you have to ask the PUK code from different places.
If the message is "SIM network unlock PIN" or "SIM ME lock code NP" or "SIM not Supported" or on an iPhone's About->Carrier Lock showing "SIM locked"
You have to ask the carrier where you got the phone from for the carrier unlock code.

Does it only ask for a PUK code when the SIM Card is in the device?
PUK/SIM Pins are set in settings and have nothing to do with Public Mobile, it's essentially locking your SIM Card so if anyone finds it they can't use it in their device without a Passcode..
I had an older referral accidentally set one on her SIM Card once, and there was no way to reset it or have CSA Provide it since they aren't the ones who put it on..
I changed her SIM Card and it fixed the issue, if you need a new SIM Card thankfully Public Mobile is running a sale on Amazon for $4.99 with two day delivery.
