Referral Forms

Since we know how many referrals seem to have been lost during most recent promo. Once dust has settled, in announcement post a simple form for us to all fill out to add the missing referrals in.

Seems better idea than taggings Mods with a new barrage of issues & leaves them free to deal with more serious issues

I also think these requests should be handled by those who handle the emails, not the moderators! It would take them away from dealing with people's "no service" issues, and since referrals are low priority, it's okay if it takes a week.
