unable to add 400 min long-distance package through self-serve

I added the money, but can't add the service.  I have tried over several days.  The $15 fee is not deducted, and I can't add the add-on package through self-serve.  Forgot my PIN from years ago, so can't do it through *611

There is a bug that is affecting the purchasing of addons.  I encountered the problem over a week ago.  They were working on resolving the issue.  I guess it is another one of those lingering problems that will eventually become a feature of the self serve system.
