Also would like to know what happens after the 90 days is over, is the "promo" still going to be available going forward or is this just for the initial 90 days? Fido has it on a month to month for the lifetime that you're activated with them. Thanks.

The promo is still active however they are not promoting anymore.
When you activate your sim and then choose to create a plan - select the Fall 2016 promo on the right side and it should automatically set your talk,text & data to the promo as well as $120 for 3 months ($40/mo)

I had the same issue when your picking the plan it doesnt say its the promo plan you need to pick the 90 days 12 gb unlimited talk txt and then chouse the fall promo on the far right it will come to $120 which looks misleading but thats for the 3 months so its good hope this helps

You can find it under "My Account" until the promo expires on Sunday.
