Unable to reactivate service

My account was suspended because I did not use it since I was out of the country for 2 months. I added $40 to my account via a recharge.com voucher, however I am still unable to reactivate my service. I have tried on multiple browsers and clearing my settings, but I still have the same issue - nothing happens when I click "submit". I have until Jan 23 to reactivate my account, so I don't understand what I am doing wrong. Any advice would be appreciated.
