You can try contacting moderators directly
https://productioncommunity.publicmobile.ca/t5/notes/composepage/note-to-user-id/22437
Give them all the info that the transfer form was asking
Good luck

The error message says to contact a moderator. Why don't you do it?
@Abnerd wrote:I have tried that, and still receive an error message. As mentioned in original post, this is Koodo monthly, not prepaid. Thanks.

@Abnerd Delete your browser's history, and then try it in Private mode(Firefox), or Incognito mode(Chrome), and give that a try. If one browser doesn't work, try the other.

@Abnerd to port Koodo prepaid, the moderators have to do it for you. Click on the green questionmark in the white circle at the bottom right of your screen. This connects to the autobot Simon. Type moderator and follow the prompts to get to one (CLICK on account specific question and human and submit a ticket)
