After a week I am still waiting for my SIM card to arrive by mail.
Any ideal what is the hold-up?

I would not have ordered the SIM if I was told of this delay.

Up to 10 business day. Some get it later, some do not get it at all. If you do not get it within 3 weeks ask agent for replacement (open ticket).

@Ducktape1 , unfortunately the SIM card fulfilment time can be quite extended, upwards of a couple of weeks.
