Probably because the 40$ went to pay your plan??? Can you go in your self-serve account and in tab PAYMENT and HISTORY PAYMENT? What can you see? Is your account shows ACTIVE in the first page?

wehen you added the voucher at that time your plan was working.second when you added r u added towards monthly bill or addons
