The customer has previously indicated that it's the SIM card's PUK is needed and was directed to a open a ticket with Public Mobile's customer support agents.

This PUK information for either the SIM card or for phone unlocking isn't information that can be found anywhere.  The PUK for a SIM card can only be provided by the carrier.  The PUK of a phone that is used for unlocking a phone to use on another network (when the unlocking PIN has been locked out of) can only be provided by the carrier that the phone is locked to (and sometimes by the device manufacturer).

Which is your phone ask for/what's the message that the your phone is displaying?
