If you haven't already, contact a PM and give them the required info.
I would imagine that even if you have already contacted them, but did not give the correct info, it would help to do so again to make sure they have all the correct info. Check out this link below to make sure you give all the right info.

I think they want you to click "get help" -> "contact us below" -> "contact us" -> select SIM & Activation and complete the form.
