LOYALTY Reward claim help.

I've had this plan since 2014 and just discovered the loyalty rewards 1$/year for max 5 years of loyalty. 

How can i claim this?????

@Sirgool if you are still on your original plan from 2014, your plan is a "legacy" plan, and not eligbile for any rewards, as noted by @mimmo.

You are probably on one of the legacy plans which are not eligible for rewards.
