For example, a PM customer has rewards in every category and switches to another carrier. After doing that and if they were to return as a PM customer for the 2nd time would all of their loyalty rewards just be reset?

If one allows their account to 'fully' close, the only rewards which they would be immediately entitled to upon a new activation with Public Mobile would be the $2.00/cycle auto-pay reward and the community rewards for participation within the Community, which runs by calendar month.
They would have to to re-build their referral rewards with new customer referrals and loyalty rewards.
This is generally why it really is unfortunate when some customers with 5+ years and a number of referrals decide it's time to leave as they're giving up some pretty sweet rewards.
However, for some, they just want a different customer service option and faster/more data.

All rewards are tied to the account. If account closed, rewards gone. And won't return.

You would be starting from scratch with your rewards, so yes they do reset because you would be considered a new user upon returning.
I would expect so. Good to know now for sure. Thanks

Accounts never get deleted, but instead simply closed. Records of it will be kept essentially forever by Telus.
