Cannot add addon

The submit button on my addon page is disable even I have sufficient fund in my account 

how can can I do it?

Try using a different browser and incoginto mode.  Clear your cache and cookies then reboot.  If you still need assistane then submit a ticket by clicking on the question mark on the bottom right.
