Add $10 referral code after activation

How can I contact customer service, I don't see any way to contact customer service to add $10 referral code and it doesn't allow me to add it. I also tried to create a ticket but it doesn't even let me submit and throws an error. Looks like the previous question I asked was removed but I can see the answer in the email which advise me that customer service can add the referral code.
 
Please advice, thanks in advance!
