Trying to port from Freedom Mobile, and got a text saying I need to fill out an activation form

There is no activation form during number Porting
But it could be a new thing after the recent sim scams. If the message is from Freedom, call freedom and get it checked.

I would get a phone number for PM first. Test the service first. After your can call in and out before porting your number to PM using the following link after login or click on the "Change Number" link on your overview page after login.

Are you trying to port your number during activation?
OR
Do you want to port your Freedom number to PM after avtivation?

There is no such form. That text message means that the porting request has failed.
Ensure that you have your Freedom Mobile account number and notice that the DBC is part of that.
Click the question mark at the bottom of the screen and then type in "rep".
