I registered a new email address in order to receive the free 5GB data special when if I activated a new phone/plan prior to Jan 15th. I activated a new phone however I can't see that the 5GB of extra data show up anywhere as available. How do I go about ensuring that I actually received the special.

If you registered the same e-mail address you used to activate, and you activated on at least a $25 or up plan, you'll get the 5 GB as @computergeek541 states within 30 days.
You'll see it here on your self-serve within a few weeks:

You'll receive the 5GB within 30 days.
