Can't successfully activate new SIM - Payment error at last step

Hi,I'm trying to activate a new SIM and the payment cannot be processed. I've tried 3 different cards. There are no fields for address or name. It's just the credit card info. How do I proceed to complete my activation?Thanks!

Thanks so much to everyone for helping. Thank you Lieux! That was a simple fix.I feel like that should be listed on the page for activations.
