did not accept my payment voucher, my sim card is activated, my register and i am on the plan but when i go to put my voucher is says 404 not found

how do i get someone to message me , i cannot do anything i have tried support , and cannot fill out all of it as i have forgot my pin and password they will not let me do anything , i have missed a payment for 60 daysv
