Couple of other thoughts.Try your sim in another unlocked phone to see if this is an account setting on the PM side.Try creating a new APN just in case there is something funny in the existing one.This is very strange. It should work.
If you want to try the port again, - Tick the I am authorized box first, only use the account #, do not use IMEI, do not use PIN and use an alternate phone number that is not the number you are trying to port.
