How did you get the offer? Email? 

The offer implies that it is only valid if you still have your old number to port back into PM. If you cannot Port in your old number, the offer is probably no good. Good luck.

@YY2002 , I assume that you have left for a different carrier brand.  Based on the description of the offer, during activation you enter the promo code at the appropriate step and you port back in the phone number associated with this special offer.  As for previously used email and password, it is not necessary but you can if you like.
