are you able to see the add on in your self-serve account? Adding adds ons here is a two step process: The first is where you add funds, the second is where you re-select the add on.
If you have funds in your account equal to the amount of the add on, re-add the US roaming package. If you do see the add on in your self-serve try restarting your phone, as well as connecting to the networks manually (T-Mobile, ect) if you havent already
