I added $28 of a data and voice add-on for 10 days, for a weekend trip to the States. I thought I had added it and it would activate when I got to the US but when I got here I didn’t have any internet or voice access. My account shows that I have $28 of “available funds” and I was charged the $28, but don’t have the plan.
anyone know what happened? And how I can get a refund for the $28 that did nothing in the end?

Buying add-on is a 2 steps process. The first step is adding money to your account. Your account has the $28 available fund. Now, you need to go back to use the same screen to activate your roaming add-on for the second step.
