SIM is PUK-locked

I accidentally turned on the PUK lock function while playing with settings and have no idea how to get back into my phone.

Apparently there is an 8 digit code that I previously created but I have zero recollection of it.

You will need to contact a CS_Agent to give you the SIM PUK.
