I was filling out the application and got to the section where I had to fill out the serial number on my phone. I accidentally pressed the continue button on that page and then tried to go back because had not completed it. It told I would need to restart the form which I thought would just mean that specific page where I was supposed to put in the phone serial number. Instead it took me back to the very beginning of the form where I had to re-input the SIN card number. When I tried to enter the SIM card number again it said it was invalid. Any advice on what to do?

@KristaPorter You get that message when your try to reenter your SIM too many times within a short period of time. The system resets after an hour.
When entering your debit/ credit card information:
1. Enter the credit card address exactly as it appears on your statement ( capitalize all if necessary ).
2. Leave the apartment field blank, if applicable enter your apt in the street address field ie xxx - street address.
3. Enter your postal code without spaces ( if there is a space on the credit card statement then enter the first 5 digits only with the space).

@KristaPorteryour sim number is still locked from your first attempt. As mentioned, try again in 1 hour when the lock will be released. When you request a port, you can enter your phone number, provider's account number and your name exactly as registered with your current provider. There is no need to enter more info.
btw who is your current provider? If it's koodo prepaid, you will need moderator help for your port request.
