@hairbag1, @bravo_md typically PUK is not a network unlock code.
@carriec77 as per the PM help articles:
A Personal Unlocking Key, or PUK code, allows you to reset your SIM card’s PIN number, after you’ve entered an incorrect PIN three times. To get your PUK code, you will need to contact the Public Mobile Community Moderator Team here.

If your cell phone is locked to a previous cell provider, you'll need to contact them for the free unlock code.

You need to get that from your last phone carrier or your phone maker (manufacturer )
