Sorry to hear you've lost your login data.
As this pertains to personal information that we on the forum do not have access to you'll need to contact PM directly. This can be done here.
I'd say
My Account -> Password -> I have a question related to my PIN or Password -> Email us
Is your best bet.

Your username is your email address used to register your Public Mobile self-serve account.
You can recover your password by using the forgotten password function here.
If you're unable to reset it, send an email to Public Mobile.
How to send an email ?
Click on this link : https://publicmobile.ca/en/on/contact-us
Then, at the bottom right of the page, click on "Email away" button.
- Choose "My Account"
- Choose "Passwords"
- Choose "I have a question related to my PIN or password"
Then click on "Email Us" and fill up the form.
