If you tried too many attempts the Forgot Password may not even work. Wait an hour, clear your browser's cache, and open a tab in incognito mode. Then try again.
If all that fails, only Moderators can help you get back in your account.
UNLESS, you have been suspended for more than 90 days. Then you lost your Public Mobile account and phone number. You would need a new SIM and activate fresh to get another account, if this is the case.

First try resetting your password at: Public Mobile - Forgot Your Password?
If that doesn't work, you'll need to open a ticket by going to: https://widget.telus.tiia.ai/publicmobile/publicmobile.html
Once there, typing in something along the lines of "forgot password".
