Are you sure you have set up an account for Self-Serve? If you didn't set it up originally, you won't have it.
Home or self activations prompt you too... retail installations usually don't get set up unless they tell you to do it, or they do it for you.
If you don't have a self-serve account created, use the "create an account" option on the right side of the "My Account" link at the top right corner of this page.

You can not create a new account for selfserve once it is linked to an email address.
Try the lost password feature. Enter the various emails you might have used. If invalid it will say we did not find that address. If valid address is entered you will be taken to next step
