Completed fields on activation screen and clicked on "activate." Got 404 error message regarding server problem. Have no idea what to do next other than throw sim card in garbage and go with a reliable carrier.

please use another computer that is fastest way to activate sim cards, sometime for no reason public mobile website show errors when activating new sim cards

@mrd1340 wait 50 minutes from last attempt, clear your browser history, go to private/ incognito mode and try again to activate your SIM card.

You can't port a number from Koodo prepaid or PC Mobile. You would have to activate with a temporary cell phone number and contact the moderators to port it.
When entering your debit/ credit card information:
1. Enter the credit card address exactly as it appears on your statement ( capitalize all if necessary ).
2. Leave the apartment field blank, if applicable enter your apt in the street address field ie xxx - street address.
3. Enter your postal code without spaces ( if there is a space on the credit card statement then enter the first 5 digits only with the space).

It is also very possible that your sim was activated but your self serve was not created, that tends to happen when the last step fails the most often.
Don't worry it's actually pretty simple, is you can't login to your selfserve, just hit the link 'register selfserve' and use the address that you used during the signup. It's better if you were not porting your number during the signup as you will need to receive an sms. You can use your old service in the meantime, unfortunately sms is not very reliable during the transfer it works off and on I noticed

I hadn't even gotten to the point of using the sim card. As a first-time customer, I was trying to register with my credit card information and that's what crapped out.

Please don't toss it yet
Check if it's working
Check if your cc was charged
If not, wait for an hour and try again
This time use incognito mode, don't use apartment field in the address nor the space in postal code, it should work
Don't forget your friend's referral code either

@mrd1340 ist your credit card was charged, then your SIM card was activated. Insert it into your phone and restart it. If you ported from another cell provider it can take up to 3 hours to complete. But you should be able to make calls and send texts.
Try to log into your self service account. If it says it doesn't exist. Create it

Did your credit card get charged, if so try rebooting your phone and see if you have any services. Your activation may have completed.
If it did, go ahead and create your self serve account:

If you did not get charged:
Try clearing out your cache, opening up a tab in incognito mode.
Then try again. You may have to wait an hour, as the SIM may need to reset, and waiting helps.
