I was bill an add on of 15 dollars on 30th April but I did not purchase it.I already have 1000 international call minutes every month with my plan and I do not need any add on.Why is this billing done automatically without my consent

I want to lock my public mobile sim card so somebody who stole my phone cannot pull out the sim card and put it in another phone to use it and retrieve my data. What is the pin I need to use to do this.
