It was scanned in. It showed service before I got the phone unlocked, after that it stopped showing service.  My account can't be set up, as no service on the phone to receive the text. Thank you though, I appreciate your help.

Phone was activated in store, validated against paperwork provided from store.

Yes. And to ensure the old SIM from the previous provider wasn't put back in. 

Yes, several times. Removed the SIM, validated it was the correct one.
