Wait, I have to pay the extra $5 every month?

I got a 53 minute incoming call after I was down to 10 minutes, and I think that is what actually took the last of the minutes. 

If I were to purchase this $5 add on, for an additional 500 minutes, is that a one time thing and they roll over every month until I use them up, or do I have to continue to pay an additional $5 mo until they are used up??
