When I bought a new sim card it did this aswell, I just used a different device to put info into. If that's not possible try an alternate browser

Contact koodo and let them know you are trying to switch your old number to your new public mobile account and then call public mobile to let them know it's been over a week

That sounds like a scam. I would call in to public mobile or you can look online in your plan area to check. Calling should be apart of your plan.
