activation failed

I was doing my activation everything is fine until I finished credit card info and hit "activate" button. System feedback msg: "error 404, ****** removed". I tried to re-do the process again but the new PM SIM card is reported invalid. Please help.

i was trying to get new number. not even get to "port number" step.

This has happened to me, but it processed.

Not sure if this was the reason why my porting did not go through.
