I would like to share my experience.
0. If you want to give your friend referral credit, it is right at the beginning. The bottom of first form.
1.You need your existing carrier's account number. If there is web based account management, you can find it. If not, you can call the customer centre. You don't need to explain why. It is not your phone number. It has to be different. It is your right to know.
2. You need the PIN of your account management log in. It is not your phone's SIM PIN or passcode. If you don't create one.
3. If you have credit in you account, find out what happen if you port the number. It is you right to know.
4. You don't need to enter your IMEI. It is not the serial number because it has to be all digit. Some serial number has alphabet.
5. It is advisible to NOT use the ported number for alternate contact. It is stated very clear on the page.
6. After the submission, check any error message.
6. Be patient. It is not very long but you may feel very long. You will received a text message to confirm the port is successful and complete.
7. Relax and enjoy.
