Your understanding is correct. The SIM card PIN couldn't possibly have even changed without the default 1234 first being entered (at least by someone even if it's someone else).

Be aware that entering a PUK incorrectly 10 times renders the SIM useless.
If you have a spare SIM you could use it to replace the current SIM by logging into your account.

@hopeboom contact customer support mods
click on the question mark chat bubble to the bottom right of your screen
type "PUK code"
(use the chat bubble options, then click "submit a ticket, click me" option)
*login using your community account
make sure to check your inbox(top right corner envelope icon) periodically, for a response

I made a few too many attempts to lock the sim and it ended up locking the phone

@hopeboom That is unlikely, unless you are attempting to lock your SM.
Reboot your phone with the SIM installed and report the exact message.
