On that website, it says to use one of 3 methods:
1. "Get the PUK code from the SIM card packaging"
2. "Sign in on your mobile carrier's website to get the PUK code"
3. "Call your mobile carrier to get the PUK code"
None of the above methods will work. The PUK is not on the packaging of Public Mobile sim cards. There also isn't any way to view the sim cards's PUK in the self serve website, and there's no way to call Public Mobile since there is no call centre.

Click on the envelope in the upper right of this window. That is the private message system.
Click on the subject line of the message to fully read it.
There you'll find a reply button.

@Noelan26 if it's a network unlock pin then contact Samsung
if it's a 8 digit sim PUK then contact customer support
click on the question mark chat bubble to the bottom right of your screen
type "PUK code"
(use the chat bubble options, then click "submit a ticket, click me" option)
make sure to check your inbox(top right corner envelope icon) periodically, for a response
