My account is on a 5G plan, but my iPhone 12 Pro only connects to LTE in multiple 5G areas. Other Public Mobile devices in the same locations have 5G. Please reprovision my line for 5G or refresh my network profile.

Check the setting in your iPhone 12. Go to Settings>Cellular Data Options>Voice & Data>5G Auto.
Also in Data Mode>Allow More Data on 5G but this will drain more data from your plan.

Hello.
I wouldn't worry if your phone says 5G or LTE. The important factor is that you're getting 5G speeds of up to 250Mbps. Do a speed test to confirm this. If the speeds are good, I wouldn't worry.
