If you need a more formal statement, you will need to request that each time you require it with CSA.

Regarding the address on file, do you mean on your Self Serve account? This address really only matters when you are matching what your registered card is connected to so payment goes through successfully. So make sure the address in the payment area, matches your card, if you have one connected to your account.
